DC Leveraging Educational Assistance Partnership Program (DCLEAP) (Cont.)
The following documents, which must be mailed or hand delivered, at the same time, are required to complete the application process for DCLEAP submission to Office of the State Superintendent of Education (OSSE). See 'Know the Required Supporting Documents' below for more information.

A twelve (12) month income/benefit history statement (for the previous year) from either of the following sources: Temporary Assistance for Needy Families (TANF), Social Security Income (SSI), Social Security Disability Income (SSDI), Retirement Income, Child Support, Worker's Compensation, Veterans Administration, or Unemployment

One current utility bill, phone bill (land line, not cell phone) bank statement, pay stub, or mortgage statement no older than 45 days from the date of the DC OneApp submission, that reflects the name and address of the applicant or their parent/legal guardian

New applicants must provide proof of high school completion (i.e. high school diploma, General Equivalency Diploma (GED) certificate, college acceptance letter or college transcript) (Current high school students may provide a signed Satisfactory Academic Progress Toward Graduation (SAP) Form with original signatures. The SAP Form can be found in the DC OneApp "Part 3: Sign & Mail Forms & Documents.").

Renewal applicants must submit an unofficial college transcript from the last semester completed.
- Affirmation and Legal Disclaimer Statement with original signature from the student and the parent/legal guardian (if applicant is a dependent) (The Affirmation and Legal Disclaimer Statement can be found in the DC OneApp "Part 3: Sign & Mail Forms & Documents.")
Note: All required supporting documents must be mailed or hand delivered to HEFS at the same time to be considered for an award.
